NAD+ boosters are heavily marketed, but the human evidence is far thinner than the advertising suggests.

NAD+ is a coenzyme essential for converting food into energy and helping sirtuin enzymes repair DNA. NAD+ levels fall with age, leading to the hypothesis that restoring them might counteract aging. The hypothesis is reasonable and worth testing — but the logic structure matters. A correlation between falling NAD+ and aging does not establish that the fall causes the aging.

What NAD+ actually does in a cell

NAD+ acts as a shuttle for electrons in the reactions that extract energy from food, making it indispensable to the mitochondria. Beyond energy production, it is consumed by sirtuins — which influence gene expression and stress responses — and by PARPs, which are central to DNA repair. Anything that increases the demand for DNA repair also draws down the available pool.

Precursors: NR, NMN, and the plumbing problem

You cannot usefully swallow NAD+ itself, so supplements supply precursors that the body converts into it — most commonly nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN). Human trials consistently show that these precursors raise NAD+ in the blood. The harder question is whether raising blood NAD+ translates into the tissues that matter, in the amounts that matter, to produce outcomes a person would notice.

Reading the human evidence honestly

The clear and reproducible finding is that precursors raise NAD+ markers. The clinically meaningful outcomes people actually want — more strength, better metabolic health, slower aging — have been inconsistent, small, or absent across studies. Some trials report a hint of benefit in a specific measure; others find no difference from placebo.

Evidence Check: Mouse data show improvements in metabolic health when raising NAD+. Human trials show precursors can raise blood levels, but clear benefits remain inconsistent and modest at best.

Free alternatives

Exercise stimulates the cell’s own NAD+ recycling. Managing total energy intake and avoiding chronic overnutrition supports the same metabolic machinery. These produce benefits that NAD+ precursors have not been shown to match — for free.

The gap between the mouse and the man

A compound that extends life in a worm or rejuvenates an aged mouse is an interesting lead, not a proven human therapy. The history of longevity research is littered with interventions that dazzled in animals and disappointed in people. Treating animal data as if it were human evidence is the central error the NAD+ marketing encourages.

A sensible posture for the curious

Treat NAD+ supplementation as an experiment with low expectations rather than a treatment with high ones. Choose a reputable product, discuss it with a doctor especially if taking medication, and be honest that any benefit may be subtle or absent. The contrast with exercise is stark: physical activity stimulates the same NAD+ pathway, costs nothing, and has decades of evidence for extending healthspan.
